About the Role

Reporting to the Operations Manager, you will deliver high-quality case management while supporting team capability and service consistency across the Client Support Partner function.

 

  • Service Delivery: Coordinate and manage the delivery of Support at Home packages, ensuring services are tailored, responsive, and aligned to client goals.
  • Client Management: Conduct comprehensive assessments, develop care plans, and support clients to navigate My Aged Care and related services.
  • Complex Case Support: Manage more complex client needs, providing guidance and escalation support to team members where required.
  • Financial Oversight: Monitor individual budgets to ensure effective utilisation of funding and alignment with client outcomes.
  • Stakeholder Engagement: Build strong relationships with clients, families, providers, and internal teams to ensure high-quality service delivery.
  • Team Support & Capability: Provide informal mentoring and guidance to Client Support Partners, supporting consistency, quality, and development across the team.
  • Quality & Improvement: Contribute to continuous improvement initiatives and ensure services align with organisational standards and compliance requirements.

 

Our Ideal Candidate

You are an experienced and confident Client Support Partner with strong case management capability and a passion for supporting both clients and colleagues. You thrive in a fast-paced environment and bring a proactive, solutions-focused approach.

 

You also bring:

  • Experience in aged care case management, including Support at Home or Home Care Packages
  • Demonstrated ability to manage complex client needs and competing priorities
  • Strong knowledge of My Aged Care and community-based services
  • Excellent communication and stakeholder engagement skills
  • Ability to mentor and support others in a team environment
  • Strong problem-solving skills, sound judgement, and attention to detail
  • Commitment to delivering high-quality, person-centred care
